Attention-Deficit/Hyperactivity Disorder (ADHD) is a highly heritable neurodevelopmental disorder characterized by persistent patterns of inattention, hyperactivity, and impulsivity that interfere with development and daily functioning. Formulated by Russell Barkley as a fundamental disorder of Executive Functioning and Behavioral Inhibition, ADHD is not a deficit of attention capacity per se, but an inability to allocate and regulate attention, working memory, and effort over time. Rooted in hypoactivation of frontostriatal dopamine and norepinephrine pathways, ADHD manifests across three clinical presentations: Combined Presentation, Predominantly Inattentive Presentation (frequently underdiagnosed in women), and Predominantly Hyperactive/Impulsive Presentation. The ADHD Psychoeducation Module provides 24 structured video lessons detailing Barkley’s executive model, adult ADHD screening, environmental scaffolding, and multimodal treatment.

Russell Barkley’s 4 Executive Subsystems

Barkley demonstrated that behavioral inhibition deficits impair four foundational executive capacities:

  1. Non-Verbal Working Memory: Holding past events in the “mind’s eye” to guide current behavior (poor hindsight and foresight; “time blindness”).
  2. Internalization of Speech (Verbal Working Memory): Deficits in the inner voice of self-governance and moral reasoning.
  3. Self-Regulation of Affect / Motivation / Arousal: Inability to generate intrinsic motivation for non-immediate tasks, requiring external urgency or high dopamine stakes.
  4. Reconstitution: Impaired mental synthesis and behavioural problem-solving.

Multimodal Treatment Framework

Comprehensive management integrates:

  • Pharmacotherapy: Stimulants (methylphenidate, amphetamines) and non-stimulants (atomoxetine, clonidine) that normalize prefrontal catecholamine signaling.
  • External Scaffolding: Externalizing time (visible clocks, timers), breaking tasks into micro-steps, and creating quiet, distraction-minimized workspaces.
  • ADHD-Adapted CBT: Addressing shame, chronic procrastination, emotional dysregulation (Rejection Sensitive Dysphoria), and executive organization.
